Community Event:

Care Improvement Plus will be on site to discuss the impact of diabetes on the health of our population then provide an overview of the resources available through a Special Needs plan. You can ask questions and receive more information as needed during the event.

This information is very important for seniors and adults with aging parents. Please help inform your communities & congregation about the health risks of diabetes and some of the ways to improve health outcomes and access more resources!
