We are currently a BILINGUAL Clinical Interventionist (RN)

Summary
Responsible for professional nursing care and related assistance to patients/families. The Clinical Interventionist is involved in direct and indirect patient care. This position provides direction to Licensed vocational nurses (LVNs), Medical Assistants (MAs), and/or Referral Specialists as well as other non-licensed nursing personnel in the provision of patient care. In collaboration with other health team members, follows the philosophy, policies and goals of LSCC.
Responsibilities:
Provide individualized Patient coaching focusing on training and encouraging patients into a state of wellness. Identify patient personal strengths in an effort to recruit them to participate in improving and maintaining their health.
Performs patient assessments/reassessments including triaging.
Teaches, supervises, and serves as a resource person and delegates in accordance with the competency level of all staff members. Serves as a mentor and completes the appropriate documentation of training and competency
Manages the referral process ensuring timely and efficient referrals to specialty services and communicates barriers to the appropriate Provider
Interprets reports, data and medical information to determine the needs of the patient/family
Interprets an extensive variety of technical instructions and manages several abstract and concrete variables
Maintains patient care standards
Coordinates with Pharmacy staff to maintain compliance and documentation for formulary, sample inventory, prescription assistance programs, prescription refills and medication management.
Complete Patient Prior Authorizations as necessary.
Performs clinical and non-clinical skills proficiently and in accordance with policies and procedures
Minimum Requirements:
Graduate of an accredited school
Valid Texas RN License
BCLS (CPR & AED) certification issued by American Heart Association or American Red Cross
Knowledge of related OSHA, CLIA, and JCAHO guidelines and requirements
Proficiency in Spanish
Computer Skills
Preferred Requirements:
Experience in Outpatient/Clinic nursing.
