HERE'S THE PROBLEM:
Texas faces a severe, multi-faceted mental health crisis. Driven by the state's highest-in-the-nation uninsured rate and widespread provider shortages, hundreds of thousands of Texans — including vulnerable youth and adults — go without adequate care. This systemic failure leads to delayed interventions, emergency room boarding, and preventable fatalities. The critical issue is “Access to Mental Health Care” and the affordability and availability of mental health care services for Texans.
The mental health statistics for Texas are staggering, but here’s a few among the many to seriously consider.
In 2025 – the 117 year old nonprofit that takes the mental health temperature for the entire United States – Mental Health America (MHA) ranked Texas as nearly last in the nation in Access to Care. In 2024, MHA ranked Texas as dead last. MHA’s next report is due in October 2026, but it doesn’t look any better than the several consecutive years Texas has ranked dead last (51 out of 50 states and Washington, D.C.).
Substance Use: About 17.7% of adults (4.3 million) in Texas reported having a substance use disorder in the past year.
Severe Youth Crisis: Nearly 1 in 5 Texas children is affected by a mental, emotional, or behavioral health condition. Approximately 75% of youth with depression in the state do not receive mental health services.
Suicide Fatalities: More than 4,300 Texans lose their lives to suicide each year. It is the second leading cause of death among youth ages 10 to 24. Suicide has steadily increased over the past two decades, according to the Texas Department of Health Services. Mental health disorders of depression, anxiety, and trauma are major risk factors.
Professional Shortages: Over 90% of Texas counties (246 of 254) are designated as Mental Health Professional Shortage Areas, meaning there are more than 30,000 residents per clinician. This shortage has left millions without adequate access to mental health services, particularly in rural areas. The state faces a projected shortfall of over 33,500 behavioral health professionals by 2036.
Uninsured and Untreated Rates: Texas has one of the highest rates of uninsured adults with mental illness. An estimated 21.4% of adults with any mental illness are uninsured. Over 62% of Texas adults living with a mental illness receive no treatment at all. An estimated one million Texans go without necessary mental health treatment each year.
Emergency System Overload: In a state where 47% of pediatric hospital stays are for psychiatric conditions, only 10% of Texas counties have inpatient psychiatric beds. This forces many behavioral health patients to board in emergency rooms.
Texas faces a plethora of challenges to become #1 in access to mental health care, and the problems and solutions are intertwined with insufficient funding, staffing shortages, and a lack of available treatment facilities.
The crisis is real.
